Fortis Entrust two-piece closed-end pouches allow the pouch to be changed without changing the compatible barrier or wafer. These beige 8-inch pouches are intended for colostomies or stomas with less frequent or more predictable output.

- Choose no-filter with belt tabs or charcoal-filter configuration

The supplier currently reports HCPCS A4424 for FTS36134 and FTS36214; A5063 for FTS36134X; A5073 for FTS36214X and FTS36234X; and A4388 for FTS36234. Coverage and billing should be confirmed for the customer’s plan.
